Report for October 30, 1941. A Douglas DC-3A (NC-21712) operated by Northwest Airlines crashed near Moorhead, Minnesota while on a scheduled air carrier flight from Chicago, Illinois to Seattle, Washington (with intermediate stops). The pilot was Clarence F. Bates, was injured in the accident. The other 2 crew members and all 12 passengers were killed.
Report for March 26, 1939. A Douglas DC-2 (NC-13727) operated by Braniff Airways crashed near Oklahoma City, Oklahoma while on a scheduled air carrier service from Chicago, Illinois to Dallas, Texas (with intermediate stops). The pilot was Claude H. Seaton. The flight hostess and 7 passengers were killed, while the pilot, first officer, and 2 passengers were seriously injured.
Report for August 30, 1940. A Douglas DC-3A (NC-21789) operated by Pennsylvania-Central Airlines crashed near Lovettsville, Virginia while on a scheduled mail and passenger flight from Washington-Hoover Airport to Detroit, Michigan. The pilot, Lowell V. Scroggins, his copilot and flight hostess, and all 22 passengers were killed. [A copy of this report is also in Folder 6]

A DC-3A (NC-16086) operated by United Air Lines crashed near Centerville, Utah while on a scheduled air carrier service from Oakland, California to Salt Lake City, Utah (with intermediate stops). The pilot, Howard M. Fey, the 2 other crew members, and all 7 passengers were killed.
Report for January 5, 1941. A Lockheed Model 18-08 Lodestar (NC-25632) operated by United Air Lines crashed near Newhall, California while on a scheduled air carrier flight from Oakland, California to Burbank, California (with intermediate stops). The pilot, William E. Davis, and his 2 crew members were not injured.
Report for October 30, 1941. A Douglas DC-3 (NC-25663) operated by American Airlines crashed near St. Thomas, Ontario, Canada while on a scheduled air carrier flight from New York City to Chicago, Illinois (with intermediate stops). The pilot, David I. Cooper, his 2 crew members, and all 17 passengers were killed.
Report for October 3, 1941. A Sikorsky S-43B Amphibion (Baby Clipper) (NC-15376) operated by Pan American Airways crashed in San Juan Harbor, Puerto Rico while on a scheduled air carrier flight from Miami, Florida to San Juan, Puerto Rico. The pilot was Charles A. Lorber. In this accident, 2 passengers were killed, 2 crew members received serious injuries, 4 crew members and 7 passengers received minor injuries, and the remaining 12 passengers were not injured.
Report for October 10, 1941. A Douglas DC-3 (NC-19970) operated by Eastern Air Lines crashed near Savannah, Georgia while on a scheduled air carrier flight between New York, New York and Miami, Florida (with intermediate stops). The pilot, Fred B. Cann, his 2 crew members and the 5 passengers were not injured.
