Contents
Report for June 27, 1942. A Douglas DC-3 (NC-17320) operated by Transcontinental and Western Air crashed near Dayton, Ohio while on a scheduled air carrier flight from New York City to Los Angeles, California (with intermediate stops). The pilot, Norman A. Hortman, his 2 crew members, and the 15 passengers were not harmed.
One (1) minor incident report from the Civil Aeronautics Board in Washington, D.C. Dated October 10, 1942.
Report for August 13, 1942. A Piper J-5A Cub Cruiser (NC-38581) and a Taylorcraft BL-65 (BL-12-65) (NC-29668) collided in mid-air near Danville, Illinois while on training flights. The pilot of the Piper, Betty Flo Beyer, was killed while the pilot of the Taylorcraft, Gilbert Frankeberger, received serious injuries.
Report for June 12, 1942. Two Piper J-3F Cubs (NC-38038 and NC-38282) collided in mid-air near Kansas City, Missouri while on training flights. The pilot of NC-38038, Hiram Leo McFarland, and his student Fred W. White were seriously injured while the pilot of NC-38282, Robert D. Livesay, was not injured.
Report on fatal accidents in September 1942 from the Civil Aeronautics Board in Washington, D.C. Dated October 15, 1942.
Report for May 12, 1942. A Douglas DC-3A (NC-21714) operated by Northwest Airlines crashed near Miles City, Montana while on a scheduled air carrier flight from Chicago, Illinois and Seattle, Washington. The pilot, Eugene S. Shank, his first officer, and a Northwest Airlines pilot, who was deadheading on the flight, were all killed. 1 passenger received minor injuries and the remaining 9 passengers and the stewardess were unharmed.
