Nine (9) minor incident reports and one (1) Safety Bulletin concerning monthly fatality statistics from the Civil Aeronautics Board in Washington, D.C. Dated July 1942.

Report for May 1, 1942. A Douglas (DC-3) DST-A (Douglas Sleeper Transport) (NC-18146) operated by United Air Lines crashed near Salt Lake City, Utah while on a scheduled carrier service from San Francisco, California to New York, New York (with intermediate stops including Salt Lake City, Utah and Oakland, California). The pilot, Donald Whittlesey Brown, his 2 crew members, and all 14 passengers were killed.
