Report for June 1, 1941. Two Douglas DC-3s (NC-25667 and NC-1941) crashed near Wichita, Kansas while operating scheduled carrier services. NC-25667, operated by Braniff Airways, was running between Brownville, Texas and Chicago, Illinois. NC-1941, operated by Transcontinental and Western Airways, was running from Los Angeles, California to New York, New York. None of the 30 total passengers and 7 total crew from either aircraft was harmed.
Report for January 4, 1941. A Lockheed Model 10-B Electra (NC-14960) operated by Delta Air Corporation crashed near Birmingham, Alabama while on a schedule carrier service from Atlanta, Georgia to Birmingham, Alabama. The pilot, Robert L. McKee, his two crew members and 2 of the passengers were not injured. A third passenger received minor injuries.
Report for November 26, 1940. A Douglas DC-3 (NC-16082) operated by Eastern Airlines crashed at the Atlanta Municipal Airport while on a scheduled carrier service from New York, New York to Atlanta, Georgia. The pilot was James J. Garrigan. No one was injured in the incident.
Report for September 3, 1940. A Douglas DC-3 (NC-19974) operated by American Airlines crashed at Hartford, Connecticut while on a scheduled carrier service from Boston, Massachusetts to New York City (with an intermediate stop in Hartford). The pilot was W. J. Cheney. The crew of 3 and 13 of the passengers were not harmed. A fourteenth passenger received minor injuries.
Report for October 23, 1940. A Sikorsky S-42B Clipper (NC-16735) operated by Pan American Airways crashed off Darrell’s' Island, Bermuda while on a scheduled carrier flight from Baltimore, Maryland to Bermuda. The pilot was W. B. Culbertson. The crew of 7 and 9 of the passengers were not harmed. A tenth passenger received minor injuries.
Report for December 7, 1940. A Douglas DC-2 (NC-13725) operated by Transcontinental and Western Air crashed outside Greensburg, Pennsylvania while on a scheduled carrier service from LaGuardia Field, New York to Chicago, Illinois (with stops in Philadelphia and Pittsburgh, Pennsylvania). The pilot was James J. Polizzi. None of the 4 people on board were injured.
Report for April 3, 1941. A Douglas DC-3 (NC-21727) operated by Eastern Air Lines crashed near Vero Beach, Florida while one a scheduled carrier service between Miami, Florida and New York City. The pilot, Gerald O'Brien, was seriously injured along with his 2 crew members and 10 of the passengers. The remaining 3 passengers received minor injuries.
Report for July 5, 1941. A Culver Cadet (LCA, LFA) (NC-34777) owned by Yakima Cloud Clippers Flying Club crashed near Yakima, Washington while on a pleasure flight. The pilot, Louis J. Roberts, received minor injuries while his passenger was killed in the accident.
Report for July 28, 1941. A Culver Cadet (LCA, LFA) (NC-34771) owned by Culver Aircraft Sales Company crashed near Dallas, Texas while on a flight to deliver a new aircraft. The pilot, William A. Gilmore, and his passenger were both killed in the accident.
Report for August 7, 1941. A Piper J-3F Cub (NC-14783) and a Waco (OH) UPF-7 (ATC #642) (NC-20979) were involved in a collision near St. Charles, Missouri while on a training flight. The pilot of the Piper, Harold Knapp, and his student were killed in the accident. The pilot of the Waco, Ellis W. Smith, was not harmed.
