Report for December 4, 1940. A Douglas DC-3A (NC-25678) operated by United Air Lines crashed near Chicago, Illinois while on a scheduled passenger and mail flight from LaGuardia Field, New York to Chicago, Illinois (with stops in Philadelphia and Allentown, Pennsylvania and Akron and Cleveland, Ohio). The pilot Philip Cramer Scott, his copilot and stewardess, and 7 of the 13 passengers were killed.
Report for March 10, 1941. A Douglas DC-3B (NC-15592) operated by American Airlines crashed near Cincinnati, Ohio while on a scheduled mail and passenger service from Chicago, Illinois to Washington, D.C. The pilot Lester W. Bryant, his copilot, the stewardess, and 9 passengers received minor injuries.
Report for April 16, 1941. A Boeing Model 247D (NC-13359) operated by PCA crashed near Charleston, West Virginia while on a scheduled mail and passenger flight from Knoxville, Tennessee to Pittsburgh, Pennsylvania (with stops in Bristol, Tennessee; Charleston, West Virginia; and Clarksburg, West Virginia). The pilot was Russell J. Wright, and the accident resulted in serious injuries to 1 crew member and 2 passengers, minor injuries to 1 crew member and 4 passengers, and 1 crew member was unharmed.
Report for January 23, 1941. A Douglas DC-3B (NC-17315) operated by Transcontinental and Western Air crashed near Robertson, Missouri while on a scheduled passenger and mail flight from Kansas City, Missouri to New York City, New York. The pilot was P. T. W. Scott. The accident was fatal to 1 passenger and 1 crew member, caused serious injury to 4 passengers and 1 crew member and minor injuries to 6 passengers and 1 crew member.
Report for February 26, 1941. A Douglas (DC-3) DST (Douglas Sleeper Transport) (NC-28394) operated by Eastern Air Lines crashed near Atlanta, Georgia while on a scheduled air carrier service from New York, New York to Brownsville, Texas. The pilot was James A. Perry. The accident resulted in the death of 5 passengers and 3 crew members, serious injury to 5 passengers, and minor injuries to 3 passengers.
Report for December 11, 1940. A Douglas DC-3 (NC-16015) operated by American Airlines crashed near St. Louis, Missouri while on a scheduled air carrier server from Chicago, Illinois to St. Louis, Missouri. The pilot was Herbert W. Susott; he and his first officer and 2 passengers received minor injuries. The stewardess and 2 remaining passengers were not harmed in the accident.
